ADHD is over-diagnosed, experts say

ScienceDaily (Mar. 30, 2012) ? What experts and the public have already long suspected is now supported by representative data collected by researchers at Ruhr-Universit?t Bochum (RUB) and University of Basel: ADHD, attention deficit hyperactivity disorder, is over-diagnosed. The study showed that child and adolescent psychotherapists and psychiatrists tend to give a diagnosis based on heuristics, unclear rules of thumb, rather than adhering to recognized diagnostic criteria. Boys in particular are substantially more often misdiagnosed compared to girls.

These are the most important results of a study conducted by Prof. Dr. Silvia Schneider and Prof. Dr. J?rgen Margraf (both from RUB) and Dr. Katrin Bruchm?ller (University of Basel) as reported in the periodical Journal of Consulting and Clinical Psychology.

The researchers surveyed altogether 1,000 child and adolescent psychotherapists and psychiatrists across Germany. 473 participated in the study. They received one of four available case vignettes, and were asked to give a diagnoses and a recommendation for therapy. In three out of the four case vignettes, the described symptoms and circumstances did not fulfil ADHD criteria. Only one of the cases fulfilled ADHD criteria based strictly on the valid diagnostic criteria. In addition, the gender of the child was included as a variable resulting in eight different case vignettes. As the result, when comparing two identical cases with a different gender, the difference was clear: Leon has ADHD, Lea doesn't.

Male and conspicuous: the "prototype" makes the difference

Many child and adolescent psychotherapists and psychiatrists seem to proceed heuristically and base their decisions on prototypical symptoms. The prototype is male and shows symptoms such as motoric restlessness, lack of concentration and impulsiveness. In connection with the gender of the patient, these symptoms lead to different diagnoses. A boy with such symptoms, even he does not fulfil the complete set of diagnostic criteria, will receive a diagnosis for ADHD, whereas a girl will not. Also the therapist's gender plays a role in the diagnostic: male therapists give substantially more frequently a diagnosis for ADHD than their female counterparts.

Diagnostic inflation, more medication, higher daily doses

In the past decades the diagnoses ADHD have become almost inflationary. Between 1989 and 2001, the number of diagnoses in German clinical practise increased by 381 percent. The costs for ADHD medication, such as for the performance-enhancer Methylphenidate, have grown 9 times between 1993 and 2003. The German health insurance company, Techniker, reports an increase of 30 percent in Methylphenidate prescriptions for its clients between the ages of 6 and 18. Similarly, the daily dosage has increased by 10 percent on average.

Remarkable lack of research

Considering these statistics, there is a remarkable lack of research in the diagnostic of ADHD. "In spite of the strong public interest, only very few empirical studies have addressed this issue," Prof. Schneider and Dr. Bruchm?ller point out. While in the 70s and 80s a "certain upswing" of studies on the frequency and reasons for misdiagnoses could be observed, current research hardly examines the phenomena. The current study shows that in order to avoid a misdiagnosis of ADHD and premature treatment, it is crucial for therapists not to rely on their intuition, instead to strictly adhere to defined, established diagnostic criteria. This is best possible with the help of standardized diagnostic instruments, such as diagnostic interviews.

Following the alpha release in December and a beta update in February for the Arc S, Neo V and Ray, Sony has pushed this version for the Play to find out which games may need some work to be compatible with Android 4.0. The usual drawbacks remain however, so because this release requires unlocking your bootloader -- which will keep users from upgrading to the final ICS update when it comes out -- and it lacks such creature comforts as Google Mobile Services apps (Gmail, Google Maps, Face Unlock) it's intended for developers only. If your gamepad-equipped handset is staying official-upgrade only, you can safely check out a video of the beta ROM running on an Arc S embedded after the break and take this as a heads up that the final release is that much closer.

Mumbai, Miami on list for big weather disasters

(AP) ? A new scientific report warns that global warming is causing more extreme storms and droughts and that the world should prepare for an unprecedented onslaught of deadly and costly disasters.

The report says the greatest danger is in highly populated, poor regions of the world, but no region is immune. The document by a Nobel Prize-winning panel of climate scientists forecasts stronger tropical cyclones and more frequent heat waves, deluges and droughts. The 594-page report released Wednesday blames a combination of man-made climate change, population shifts and poverty.

The scientists say that some places, particularly parts of Mumbai in India, could become uninhabitable from floods, storms and rising seas. Other cities at lesser risk include Miami, Shanghai, Bangkok, China's Guangzhou, Vietnam's Ho Chi Minh City and India's Kolkata.

(AP) ? An AWOL soldier's simple scheme to defraud one of the richest men in the world has landed him in federal custody, according to a criminal complaint.

In the complaint unsealed Monday, federal investigators allege Brandon Lee Price changed the address on a bank account held by billionaire Microsoft co-founder Paul Allen, then had a debit card sent to his Pittsburgh home so he could use it for payments on a delinquent Armed Forces Bank account and personal expenses.

Price called Citibank in January and changed the address on an account held by Allen from Seattle to Pittsburgh, then called back three days later to say he'd lost his debit card and asked for a new one to be sent to him, an FBI investigator wrote in a criminal complaint filed in February.

The card was used to attempt a $15,000 Western Union transaction and make a $658.81 payment on the Armed Forces Bank loan account the day it was activated, according to the complaint. Surveillance footage also captured him attempting purchases at a video game store and a dollar store, authorities alleged.

Investigators found Price was listed as Absent Without Leave from the Army and wanted as a deserter, authorities said in the complaint. He was arrested March 2 and ordered detained until April 2 unless the Army takes him into custody.

David Postman, a spokesman for Allen, said the fraud was detected by the bank, who alerted law enforcement officials. The only transaction ? out of four listed in the complaint totaling $15,936.99 ? that apparently made it through was the loan payment, Postman said.

While the financial loss is comparatively small, it shows the importance of the issue of identity theft prevention.

"Clearly, it's a reminder that anyone can be a victim of this," Postman said.

The complaint doesn't specify how Price allegedly changed the address on the account. A phone message left for Citibank was not immediately returned Tuesday.

An email to Price's public defender wasn't immediately returned, nor was a voicemail left at a phone number for Price listed in the complaint.

Allen made the bulk of his fortune founding Microsoft with Bill Gates in 1975. He left the company in 1983 and now owns the Seattle Seahawks and the Portland Trail Blazers. He is the founder and chairman of Vulcan Inc., the company that manages his business and charitable undertakings. Forbes estimates Allen's net worth at $14.2 billion.

Accounts with other credit card companies may have also been targeted, but none of Allen's other accounts were compromised, Postman said.

"It certainly is a surprise and reason for everyone to make sure that all that stuff is properly cared for and monitored," Postman said.

Just under 280,000 cases of identity theft were reported in 2011, according to the Federal Trade Commission's Bureau of Consumer Protection. That includes complaints through the Better Business Bureau and state attorney general offices and other law enforcement agencies.

MBA: Mortgage applications continue 6-week drop - Gay Real Estate

Mortgage applications declined 7.4% for the week ended March 16, as refinancing hit its lowest share of loan activity since July 2011.

The Mortgage Bankers Association seasonally adjusted index fell for the sixth straight week, with the refinance measure down 9.3% in a report Wednesday. Refinancings made up 73.4% of all mortgage apps, down from 75.1% a week earlier.

The purchase-mortgage index dropped 1% from the week prior.

The trade group?s four-week moving average fell 2.79% overall, with purchase apps up 3.25% and refinancings down 4.31%.

The drop in refinancing apps, however, doesn?t reflect huge increases of Home Affordable Refinance Program requests in some states, according to MBA Chief Economist Jay Brinkmann. He said all refinancing apps climbed in February from January in Nevada by 71%, Arizona by 61% and Florida by 49%.

?Refinances in the rest of the country were generally flat or even down,? Brinkmann said in a release. ?HARP clearly is a driving force in those states that saw the most defaults and the biggest drops in home equity.?

Completed HARP refinancings dropped to a 25-month low in December, according to the Federal Housing Finance Agency. An agency spokeswoman said changes to the program that increased eligibility might not show until the release of first-quarter data.

The average interest rate on a 30-year, fixed-rate mortgage for a conforming loan rose to 4.19% from 4.06% a week earlier, according to the MBA. Rates on 30-year FRMs backed by the Federal Housing Administration increased to 3.93% from 3.82%.

The average purchase mortgage increased roughly 4% to $225,463 in February from $216,888 a month earlier, while refis dropped 2.4% to $222,048 from $227,563.

On Tuesday, March 19th, Alan Jacobs posted a technology article for The Atlantic titled ?Jobs of the Future: A Skeptic?s Response.? In the article, he voices his doubts that a skillset promoted by the internet and social networking would usher in a new wave of future employment:

?Where are these jobs that will?require such rapid ?searching, browsing, assessing quality, and synthesizing the vast quantities of information?? We don?t need those skills to drive a truck or manage company accounts or sell clothes or do IT customer service or write novels or write code or give inoculations to patients or teach seven-year-olds how to read ? so what do, or what will, need them for? And how many of us will need them??

We might not need those skills to drive a truck, but if you are responsible for a whole fleet of trucks, you may need to search a database that tracks every truck?s location and cargo. Online retailers of clothes have huge customer databases that they can use to selectively target promotions to those they think would be most responsive to them. A typical call to IT tech support goes to a first tier agent, whose first task is to request your information so they can find you in a database. Then, when it comes down to the actual support, if they don?t know how to fix your problem, they search through forums, help documents, and bug databases to try and find an answer.

In my own field of bioinformatics, the trend toward ?searching, browsing, assessing quality, and synthesizing the vast quantities of information? is driving a shift of focus (more on that in a future blog post) that is real science, not science-fiction.? The advancement of machines that sequence DNA has progressed so rapidly that it?s broken Moore?s Law , and we can now sequence an entire human genome in a ?matter of weeks?.

All of these facts point to a single trend: our ability to produce data is outstripping our ability to understand it. In fact, the need to make sense of these mountains of information is so great that it?s given rise to one of the hottest interdisciplinary fields on the market: data mining and predictive analytics.

Data mining, loosely defined, is the process of analyzing data and shaping it into useful information. But how does a data miner know which piece of data matters and which doesn?t? They must cleave through those mountains of information in order to find data diamonds, and that requires the skills that Alan Jacobs questions.

Later in his article, he calls on employers of the future to seek individuals capable of slow, patient, and careful thought. Rest assured, these skills will not fall by the wayside. Let?s set aside the fact that we?ll always need this kind of thought to drive innovation. This kind of ?deep thought? (and no, I don?t mean the Jack Handy variety) is the next logical step after data mining, and it is called predictive analytics. Using a statistical skillset, analysts try to predict future trends in everything ranging from the business forecasting to political unrest.

In the field of bioinformatics, we produce more genetic sequence data than we can analyze. The fact is that before we can do any deep thinking about this data, we have to mine out the really difficult problems, to separate the wheat from the chaff, so we can turn them into diamonds. I suspect this trend applies to just about any field that collects data, and what field in this century doesn?t? As the clich? goes, knowledge is power, and this is one clich? that proves truer day by day.

So what do jobs of the future look like? Yes, we?ll still need truck drivers, teachers, and novelists. I won?t argue that their core competencies will include data searching, though they can all benefit from these skills. For the growing workforce that deals with data, however, these skills are mandatory, and of those people, it will be the deep thinkers who make that data precious.

Robert LoCascio: Great Person #2: The Fragility of Life, Dreaming Big, and Ending Our Dependence on Oil

Shai Agassi is on a mission to make a big dream come true. At the age of 33, he sold his first software company for a few hundreds million dollars to SAP. He then went on to lead SAP's overall technology strategy and execution before he was designated as next in line for the role of CEO. Then, as the story goes, he decided to take a left turn and continue his journey on a completely different path -- a path that more closely aligned with a personal passion. He decided to build an electric car company that could potentially end our dependence on oil. The name of the company is A Better Place and he recently opened his first location in Israel. He is slated to open additional locations in Australia, China, Denmark, Japan, Canada, California and Hawaii. I caught up with Shai at his first showroom, located in Israel, to take a test drive and to interview the person behind this great vision. By the way, the cars are really fast and make no engine noise.

Robert: What's your purpose in life?

Shai: To end oil!

Robert: Did an event or person in your childhood change the way you think? For instance, in my life, my grandfather always said, "Don't be a follower."

Shai: I had lots of different events in my life. I went to college at a very early age and towards the end of my bachelor's degree, a car hit me and I ended up in the hospital for almost a year and a half. It took four painful surgeries to recoup my ability to walk. This accident taught me how fragile we are. While you've got to be focused on what you are doing, you still need to remember that everything can change at any moment in time.

Robert: Was there one failure that gave you your greatest lesson?

Shai: Just like most start-ups and entrepreneurs, at one stage of my life, I had a company that was writing software for other people and we had five contracts that we were working on at the same time. Then all five contracts, independently of one another, got cancelled within forty-eight hours. We suddenly had only two weeks of cash flow left.

Robert: What happened?

Shai: It was the best thing that ever happened. It focused me and I basically decided to stop building stuff for other people and decided to build my own product. That turned us from a service company to a product company and as a result, eighteen months later, we sold the company for $110 million. So we went from bankruptcy to success within a short time.

Robert: I had a similar experience with my company in 1998. We were building websites and I lost my largest customer, was running out of cash and then decided to build a product. We went Public on NASDAQ 18 months later after that incident with LivePerson. I often think there is some strange way in which the world works, that when you are faced with a serious crossroad, if you cross it, great things can happen.

Robert: So if you wrote your obituary, what would you hope it said?

Shai: He dared to dream.

Robert: Is there a person in your life who you really admire, who is a hero to you?

Shai: My girlfriend.

Robert:
Why?

Shai: There are too many things to describe. She's just got the ability to make everybody around her better, aspire to be better, and she's always got my back.

Robert: What are your core values?

Shai: Wow. Let me describe a Better Place's values as I think it's better than describing my core values. Our values are dream big, run responsibly, invite everyone, value our people, execute with excellence.

Robert: If you could go back and change one day, which day would you change?

Shai: I know what day it is, but I cannot tell you. It is proprietary!

Robert: What would have to happen in the world to make the world a perfect place for you?

Shai: I think we need more people who are able to take big problems like oil, the environment, healthcare, education, and dedicate themselves to solving them. Not just from a passion side, but also from an engineering side of creating new systems. If we found twenty more big thinkers, who were willing to execute and dedicate their lives, I think the world would become truly a better place.

Robert: If you could get one question answered that I haven't asked you, what would that question be?

Shai: When do you think no one will buy a gasoline car in Israel? The answer is before the end of the decade!

Robert: Ok. Last but not least, is there some story that you would like to tell that tells your story?

Shai: It's been told many times. I presented the whole idea of a Better Place as a white paper back at the end of 2006 during a conference and Shimon Peres, who is the President of Israel. He took my white paper and then three days later he called me into his office. What I didn't know is he showed the white paper to many experts and everyone said I was crazy. He said, "If everybody thinks you are crazy, you must be leading the market and the world." Within two weeks, we had an agreement with the government of Israel to turn my white paper into a reality. A month later, we had an agreement with Renault to make the cars and a year later we launched the company. Those kinds of moments are truly rare. It's so much easier for political leaders in the world to basically look at big ideas and not want to put their name on it. It's very rare to find a political leader that's willing to step up and say -- We've got to make something happen and we're going to take a risk on a big new idea. I guess, sometimes it takes the crazy guy to make things a reality.

Robert: Awesome.

SHENZHEN, China?The supercomputer in this southern boomtown is named Nebulae for the interstellar clouds of gas that give birth to stars. The machine symbolizes China's soaring ambition to challenge the U.S. and other developed nations in technology, but also underscores the limitations of what China can achieve.

China's unexpected progress in developing supercomputers, the brains of modern science and an engine of economic development, has caused an outbreak of anxiety over the past two years in the U.S., which has long been the field's undisputed leader. China's advances in computing have been critical to its ability to build spacecraft and advanced warplanes and to its growing prowess in genetics. China overtook Japan in 2010 as the number two investor in research and development, according to Battelle Memorial Institute, a Columbus, Ohio, research outfit. Though China remains well behind the U.S. in R&D spending, it is gaining ground.

But a closer look at China's supercomputers reveals a program that is far less of a threat to U.S. technological dominance than commonly believed. Chinese researchers say decisions about how supercomputers are used are often made by local politicians more interested in local development projects than breakthrough technology.

China's bureaucrats meanwhile haven't figured out how to mount software development projects that come close to U.S. or European standards. Chinese scientists also lack the funding, and freedom, to explore technologies that haven't already been endorsed by the government, which can keep them well behind the cutting edge.

The result is that China's supercomputing projects aren't producing the kinds of breakthroughs that can create new industries. It is instead being deployed to help the country simply catch up with the U.S. and Europe, in areas ranging from health care to automotive design to aviation. That is important economically, but it is also a reminder that China remains a developing country whose main goal is to close the economic and technology gaps with richer nations.

"The strategy has been never to lead, but to follow" technologically, said Qian Depei, a Beihang University researcher, who has worked for decades on China's advanced computing programs. "That was the most economically efficient way to develop."

Richard Suttmeier,a University of Oregon expert on Chinese science policy, said China hasn't figured out "the right formula" to pioneer new technologies in part because researchers are rewarded according to the number of academic papers they publish rather than the quality and novelty of their work.

Supercomputers are largely seen in China as local economic-development tools. City governments play a much larger role in setting China's supercomputer research agenda than they do in the U.S. because Chinese cities finance a larger share of the projects.

Shenzhen, which paid three-quarters of the $1.3 billion cost of the Shenzhen supercomputer center, "doesn't care about climate change and astrophysics"?traditional supercomputer research projects?said Feng Shengzhong, deputy director of a Shenzhen research institute that develops applications for the Nebulae. "They care about local problems."

He is working on a plan to use the Nebulae to improve health care services in South China?a socially important goal but not one that makes use of the power of what is ranked as the world's fourth-fastest supercomputer.

China is now home to 74 of the world's 500 fastest supercomputers?which can make trillions of calculations every second?up from just 10 in 2007. Changes in the way the machines are designed have helped the country.

In the 1980s, when Cray Research in Minneapolis was the world's supercomputer technology leader, the machines were powered by a few enormously powerful processors, whose design was difficult to match. Exports were tightly controlled. Starting in the 1990s, supercomputer researchers began to lash together tens of thousands of off-the-shelf microprocessors to work on a single job. China could buy those computer chips from Intel Corp., Advanced Micro Devices Inc. and other firms and make its own machines.

Analysts say that Chinese scientists have benefited from training at top computer centers in the U.S. and Europe and the availability of computer chips and other parts from abroad, as well as consistent Chinese government funding and support. The supercomputer effort isn't dogged by charges that the Chinese have ripped off foreign technology. Rather, Chinese scientists say U.S. restrictions on some high-technology exports have required them to redouble their domestic efforts.

Still, China remains largely dependent on U.S. made microprocessors?the brains of the computer?which puts it behind the leading edge. Beijing has developed one computer that uses locally designed microprocessors, but doesn't run commercially available software. Another microprocessor in development, the Loongson, would use existing software and could eventually become a competitor to Intel and others.

Some of China's supercomputers have been used to design wings for China's stealth fighter, now in test phase, and to design parts for China's first commercial jet. Beijing lags well behind the U.S. in both efforts.

In Shenzhen, the Nebulae, which is still being tested, is expected to improve storm warning systems and help genetics companies search for the causes of disease. But it is also scheduled to be used for far less demanding tasks, such as processing video animation.

In the U.S., said Steve Conway, a supercomputer analyst at market researcher IDC, in Framingham, Mass., cities and states chip in money for local supercomputer centers, but they generally have little say in setting the priorities for the machines. U.S. supercomputer centers nearly always focus on advanced scientific research, such as designing drugs tailored to individuals.

Research at the edge of technology is risky, but can have big payoffs and leave competitors like China well behind. "American alarmism isn't always well-founded," said Mr. Suttmeier, of the University of Oregon. "The critical point is keep devising strategies in the U.S. to stay way ahead of the game."

One of China's greatest weaknesses is in software development, a potentially crippling problem because the usefulness of the machines depends on the quality of the software applications. Less than 10% of supercomputing funding goes to developing such applications, said Chinese researchers who complain that political leaders press them to build headline-grabbing new machines rather than focus on whether they are used to their full capabilities.

In the U.S., which spends about six times as much on supercomputers as China, the software budget equals about 30% of hardware spending, and computer specialists say even that level isn't sufficient.

The battle for software dollars is so intense in China that researchers rarely work as a team on long-term software projects, Chinese scientists say.

To illustrate the uneven perception of China's supercomputer efforts, Mr. Qian, the veteran supercomputer researcher, holds his palms at hip level. "Generally, we're here," he said, "but everyone thinks we're higher," as he raises his palms to shoulder height.

New search launched for missing tot Ayla Reynolds

Police have told Trista Reynolds the 'significant amount' of blood found in the home where her missing daughter Ayla was last seen is indeed Ayla's.

Updated at 5:455 p.m. ET: With the ground clear of winter's snow,?crews on Saturday renewed the search for missing toddler Ayla Reynolds in the woods and fields of Waterville, Maine, but failed to turn up any new clues.

The girl vanished from her father's home in Waterville in December.?Ayla was 20 months old at the time.

Police are treating her disappearance as a crime, but no one has been charged.


Maine Department of Public Safety spokesman Steve McCausland said?the estimated 100 searchers included volunteers, firefighters and game wardens.

They were being aided by six tracking dogs, he added.

Seven-weeks after an infant went missing while in her father's care, police in Maine have resumed their underwater search for Baby Ayla. NBC's Michelle Franzen reports.

Searchers targeted places that were not thoroughly searched over the winter when the ground was covered with snow.

"The weather conditions were ideal, and the snow's gone, so that's why the ground search was resumed today," McCausland said.

McCausland also asked people in?greater Kennebec County "to keep an eye on their own property for any signs that could be related to the case, now that the snow has melted."

Ayla was living with her father, Justin DiPietro, when she disappeared. She?was reported missing on Dec. 17 when her father said he found her bed empty. DiPietro has said he has no idea what happened to his daughter or who is responsible.

TODAY legal correspondent Savannah Guthrie talks about the developments in the case of missing Maine toddler Ayla Reynolds and why police waited several weeks until revealing discovery of blood.

Police have received 988 tips, McCausland said.

Detectives were continuing to conduct interviews and?the crime lab is providing new evidence almost daily, he said.

This matchup features another four versus one seed matchup. The Kentucky Wildcats have everything going for them. They?re healthy, they have arguably one of the best big men to appear in the NCAA in a generation in center Anthony Davis, and are exceedingly well-coached by John Calipari, who?s won 77% of games in his career, and is the only coach in NCAA history to take three one-seeded programs to the NCAA tournament (the others being the University of Massachusetts and Memphis).

Home alone -- depression highest for those living alone

The number of people living on their own has doubled, over the last three decades, to one in three in the UK and US. New research published in BioMed Central's open access journal BMC Public Health shows that the risk of depression, measured by people taking antidepressants, is almost 80% higher for those living alone compared to people living in any kind of social or family group.

For women a third of this risk was attributable to sociodemographic factors, such as lack of education and low income. For men the biggest contributing factors included poor job climate, lack of support at the work place or in their private lives, and heavy drinking.

It is known that living alone can increase the risk of mental health problems for the elderly, and for single parents, but little is known about the effects of isolation on working-age people. Researchers in Finland followed 3500 working-aged men and women for seven years and compared their living arrangements with psychosocial, sociodemographic, and health risk factors, including smoking, heavy drinking and low physical activity, to antidepressant use. Information on antidepressant medication was taken from the National Prescription Register.

Dr Laura Pulkki-R?back, who conducted the research at the Finnish Institute of Occupational Health, explained, "Our study shows that people living alone have an increased risk of developing depression. Overall there was no difference in the increased risk of depression by living alone for either men or women. Poor housing conditions (especially for women) and a lack of social support (particularly for men) were the main contributory factors to this increased risk."

She continued, "This kind of study usually underestimates risk because the people who are at the most risk tend to be the people who are least likely to complete the follow up. We also were not able to judge how common untreated depression was."

While this study clearly identifies some of the factors which increase the risk of depression for people who live alone, over half the increase in risk is still unexplained. The researchers suggest that this may be due to feelings of alienation from society, lack of trust, or difficulties arising from critical life events. All these factors need to be addressed in order to begin understanding and reducing the incidence of depression amongst working age people.
